Used glass plate (standard slide size) that contains a crosshair pattern in the center like pictured below (dimensions are in mm).

It was marked by previous owner, but you can just replace the paper label and write new values for your microscope.

Also, you may want to clean it a bit, though this isn't necessary - pattern is clearly visible through a microscope.
